        Welcome to the Application. These terms and conditions are important and affect your legal rights. Professional Laboratories Management Corp. Public Company Limited will notify the change, amendment or deletion of the Terms and Conditions of the Application Service on the Application in advance and request confirmation that you accept such changes again. Such changes will be effective when you confirm your acceptance of such changes and shall be considered as part of the Terms and Conditions of the Application Service. If you do not accept, stop using the Application.

  • "Application" means the Application under these Terms and Conditions.
  
  
  • "Service Provider" means Professional Laboratory Management Corporation Public Company Limited and persons assigned by the Service Provider to oversee the provision of the Application Service.
  
  
  • "Personal Data" means information about the personal characteristics of the administrator, such as name, surname, national identification number, mobile phone number, personal data and other information that contains the name of the person, number, code or anything that can identify that person, photographs that are in the database system of the service provider and / or that the service provider has collected from the legal representative, other persons or agencies for the benefit of the service provider, which is protected under the law on any action on the computer network and any personal data protection law.
  
  
  • "Device" means any device used to use the Application, such as a mobile phone, tablet, computer, or any other device or tool specified by the Service Provider for use of the Application.

  • This application allows legal representatives to register as legal representatives with personal information and photos of the research participants, which allows the administrators to access the research participants' information, which will be linked to the database system of Professional Laboratory Management Corp. Public Company Limited, in accordance with the terms and conditions set by the service provider.
  
  
  • The use of this application is free of charge. The service provider's provision of the application does not include the provision of equipment to use the application. If the user wants to use the application, they must be connected to the internet and telecommunications network. The provider will not be responsible for any costs incurred.
  
  
  • The purpose of each menu/function usage will appear in the application, and the information that appears in the application is for use only.
  
  
  • Any action in which the administrator registers as a legal representative on the application is considered to be reliable and the legal representative has given consent for the results of such action to be complete, correct, and can be used as evidence in all respects, including accepting responsibility for such action.
  
  
  • The user shall not disclose and/or transfer the patient's personal information to any person or use the patient's information from the application for any purpose other than that specified.
  
  
  • Users shall not use the application for purposes that are against the law, public order or good morals.
  
  
  • The use of certain functions on the application in accordance with these terms and conditions may have other terms and conditions that appear depending on the use of such functions. The user is responsible for understanding each term and condition and can decide to use the function at the discretion of the administrator. However, the terms and conditions of use of each function are not considered part of these terms and conditions of service.
  
  
  • If there is an error in using this service or there is a question about the user's information, the user must notify the service provider of the relevant details so that the service provider can use the information to check the error.
  
  
  • The service provider is not responsible in any case for any damage or malfunction resulting from the use of the application and/or the use of any device by the research project participants.
  
  
  • The service provider is not liable to the user in case of any damage or loss arising from registration or use of the application.

  All information on the Application is the intellectual property of Professional Laboratory Management Corp. Public Company Limited. The content of the information on the Application and the design of the Application, including text, graphics, software, photographs, videos, creativity, selection and arrangement, as well as the source code of the Application, are the property of Professional Laboratory Management Corp. Public Company Limited.



• Application deactivation
  
  
  • The service provider reserves the right to not provide the service or stop providing any or all parts of the application at any time with prior notice.
  
  
  • The Provider reserves the right to cancel these Terms and Conditions or revoke any or all rights of the Research Participant that the Research Participant has received under these Terms and Conditions at any time with prior notice.
  
  
  • Participants in the Research Project acknowledge and agree that termination of use of the Application and termination of these Terms and Conditions, including the termination of any modified Terms and Conditions, shall not affect any legal rights, obligations, liabilities or legal obligations.


• Applicable Law
  
  

  If any dispute arises from the service of the service provider due to this service, the legal representative and the service provider agree to use Thai law to resolve the dispute.
